Complex Posterior Fossa Surgery for Chiari I Malformation Successfully Performed at Ruby General Hospital A technically challenging surgery for Chiari I Malformation with significant cervical syringomyelia was successfully performed at Ruby General Hospital, Kolkata, by Consultant Neurosurgeon Dr. Biplav Singh. The 35-year-old patient had been experiencing symptoms for nearly two years, including persistent occipital headache, weakness and reduced grip strength in both hands, balance and walking difficulties, and sensory disturbances. Imaging revealed Chiari I malformation with cervical syringomyelia and C1 vertebral assimilation, making the surgical anatomy particularly complex.An extensive neurosurgical procedure was performed, including foramen magnum decompression, C1 laminectomy, restoration of normal cerebrospinal fluid (CSF) flow, and lax duroplasty using a vascularised pericranial graft. The surgery aimed to relieve pressure at the brain–spinal cord junction and restore normal CSF circulation, addressing the underlying cause of the syrinx. Despite the complex anatomy, the procedure was successfully completed.
