💉 Once-weekly insulin is here — but let's clear up the biggest myth around it. Many patients hear "one injection a week" and assume ALL their insulin is now down to once a week. That's not correct, and it's important to understand why. Once-weekly basal insulin (like insulin icodec) is designed to replace only your long-acting basal insulin — the background dose that keeps blood sugar steady through the day and night. Instead of a daily basal injection, you take one shot, same day every week. What it does NOT change: if you're on a basal-bolus regimen, your mealtime (bolus/prandial) insulin requirement stays exactly as it is. Bolus insulin covers the sugar spike from food, and it still needs to be taken with meals, based on your carbohydrate intake and blood sugar readings — once-weekly dosing simply doesn't apply to it. So for someone on basal-bolus therapy, the change is: 7 basal injections a week become 1, while mealtime insulin injections continue exactly as before. This can genuinely improve convenience and adherence, especially for people who struggle with daily basal dosing — but it's not a "one shot replaces everything" solution.
