Patellofemoral Pain Syndrome (PFPS) Anterior knee pains commonly referred to as "Runner’s Knee," is a medical condition characterized by pain at the front of the knee and around or behind the kneecap (patella). It occurs when the kneecap rubs against the thighbone (femur) abnormally, leading to irritation of the cartilage and surrounding tissues. Key Symptoms Aching Pain: Dull, aching pain at the front of the knee or directly behind the patella. Activity Triggers: Pain that intensifies during weight-bearing activities on a bent knee, such as running, squatting, jumping, or navigating stairs. "Theater Sign": Discomfort or stiffness after sitting with knees bent for extended periods (e.g., in a cinema or during long drives). Crepitus: Grinding, popping, or clicking sensations in the knee during movement. Primary Causes & Risk Factors Overuse & Load: Repetitive stress on the knee joint without adequate recovery time, often caused by sudden increases in training volume, intensity, or distance. Muscle Imbalances: Inadequate strength or flexibility in supporting muscle groups, such as weak quadriceps (especially VMO) or hip abductors, and tight hamstrings. Biomechanics: Structural misalignment causing improper patellar tracking in the femoral groove, including overpronation (flat feet), wide Q-angle, or knees collapsing inward (valgus).
