Complex PCI After Previous Bypass Surgery A 65-year-old woman with diabetes, hypertension, previous CABG, heart failure, and acute kidney injury on chronic kidney disease presented with worsening cardiac symptoms. Coronary angiography revealed severe three-vessel disease, critical narrowing of the SVG–RCA graft, and a long, heavily calcified LCX lesion. Using an individualized complex PCI strategy with careful lesion preparation, GuideLiner-assisted stent delivery, and advanced interventional techniques, both the SVG–RCA graft and calcified LCX lesion were successfully treated with good final blood flow. This case highlights how selected high-risk patients with previous bypass surgery and complex coronary artery disease may still benefit from carefully planned PCI with appropriate heart–kidney risk management.
