​🩺 Clinical Case Share: Managing Massive PR Bleeding in Radiation Proctitis​Managing severe, acute lower gastrointestinal bleeding requires rapid stabilization and precise endoscopic intervention. This was highly evident in a recent case involving a patient with a known history of chronic Radiation-Induced Proctitis (RP) who presented with massive per rectal (PR) bleeding.​🔍 The Clinical Challenge​Chronic radiation proctitis is a challenging sequela of pelvic radiation therapy. The tissue becomes fragile, characterized by diffuse, superficial neovascularization and telangiectasias that can bleed profusely. When a patient presents with a massive hemorrhage from these lesions, achieving rapid, widespread hemostasis without causing further deep tissue necrosis is critical.​⚡ The Intervention: Spray APC​To tackle the diffuse nature of the bleeding, we utilized Argon Plasma Coagulation (APC) via the non-contact "spray" technique.​Using a non-contact, sweeping motion allows the ionized argon gas to seek out and systematically coagulate the superficial, fragile vessels across a broad surface area. This approach minimizes the risk of tissue perforation or deep thermal injury to an already compromised rectal wall.​The procedure was highly successful: immediate and complete hemostasis was achieved, and the bleeding was controlled.
